1. Guarantee: We at TLC believe totally in our craftsmanship & quality of materials that we guarantee all our work for a period of FIVE YEARS from the laboratory delivery date, if any restoration fails due to break down or poor design, we will repair of replace it at no charge. (Porcelain veneers are exempt.) Warranty becomes nullified if patient does not receive their annual check-up, breakage due to trauma or restoration failure due to oral health problems.
2. A 2% discount will be applied to your account when full invoice payment is received within 10 days of invoicing. This will be credited to your following months invoice.
3. All accounts are payable within 30 days of invoice.
4. All items supplied remain the property of the laboratory until payment is received.
5. The company reserves the right to check the credit rating of any client.
6. The company will charge interest any account that becomes 60 days overdue at a rate of 2% per month which will be applied to the total amount outstanding including any previous interest charged.
7. Any legal fees or other costs required to collect an unpaid balance will be charged to the customer.
8. Accounts with balances unpaid for over 60 days will be processed only on a pay first basis.
9. No claims for credit will be considered after 21 days from the date of invoice.
10. A loss and handling charge is added to the weight of gold and precious alloys used.
11. Appliances are constructed to the specification prescribed on the laboratory prescription. The laboratory is not responsible for the suitability of that specification.
12. By fitting an appliance the client accepts that the laboratory has produced the appliance to the specification and satisfaction of the client.
13. All prices are subject to alteration without prior notice.
14. The client is responsible for any additional costs or charges incurred through changing instructions or delivery dates after the work has been accepted by the laboratory.
15. The laboratory will only consider claims for remakes at the concessionary remake rate within 30 days of the delivery date.
16. The laboratory reserves the right to charge for the remaking of an appliance. A product (crown/bridge/implant/ mouthguard etc) that fits the master model poured from the original impression supplied and is fabricated to the specifications on the prescription, but subsequently has to be remade is NOT considered a remake.
17. The laboratory will charge for Express or Sprint services for crown and bridge cases where the delivery date specified by the client determines that either of these services will be required to enable delivery of the work on the date specified.
18. The laboratory endeavours to interpret and follow the client’s instructions correctly. If, however, a mistake is made, the laboratory will rectify the situation free of charge as quickly as production techniques allow. The laboratory will not compensate dentist’s fees, consequential losses or expenses of any kind.
19. The laboratory endeavours to deliver work on or before the delivery date specified by the client. If, however, for any reason this has not been possible, the laboratory will not compensate dentist’s fees, consequential losses or expenses of any kind.
